Example 1 - copy and paste a new layer, still looks the same,

then desaturate and turn the contrast up full,

magic wand a black part, select similar, contract the selection (by 2 here), I use the gradiant tool with background and forground both white,

Mergency II fades in the background nicely.
the difference between DCMergency and Mergency II is this one is oval, the other is square.

Example 2 - same background,

use something else for the layer,

unsee the layer momentarily,

select the victem,

resee the layer,

and Mergency II pulls up the background in a nice oval shape.
